∙ 대다수에서 치료는 6주간의 DIP extensor splinting으로 치료한다

∙ 자유롭게 움켜잡는 것은 적어도 8주간은 금해야 하고, full flexion으로 회복은 몇 개월이 걸린다
∙ DIP jt.를 hyperextension으로 고정화시키는 것은 DIP jt.를 덮고 있는 skin의 ischemia를 유발하여 necrosis까지 생길수 있으므로 피해야한다
∙ 수상후 한달이 지나서 splinting으로 …(drop)

Repair of the Extensor Tendon System
Digital extensor system의 terminal insertion인 lateral band의 attachment 부위가 disruption되면 DIP jt.의 active extension이 소실되고 distal phalanx가 꺽이게 된다
→ “mallet finger” deformity
∙ 대부분의 DIP jt.의 extensor tendon의 disruption은 closed type이고, 일상생활에 흔한 finger tip에 axial blow가 가해질 때 생기며, 그 즉시 flexion을 유발시킨다
∙ pain은 적다
∙ PIP jt.를 extension시키고, lateral band와 oblique retinacular lig.가 distal로 활주하는 것을 예방한다
∙ partial이나 complete한 tendon rupture로 그에 해당하는 만큼의 active DIP extension을 상실할 것이다.
